Your First Visit to Minnesota Head & Neck Pain Clinic

Before your visit

Becoming a new patient is simple with our online portal. After scheduling your appointment by phone, you will receive an email with instructions on how to register online before your visit. Completing your online registration and pre-check-in will help ensure a seamless experience when you arrive, allowing you to make the most of your time with the doctor. You can complete the online pre-check-in and fill out your health history through the patient portal 7 days or less before your appointment. If you prefer, you can download, print, and complete the New Patient Registration Packet by clicking the New Patient Packet link below.

What to bring to your visit

To ensure you get the most out of your visit, please bring the following with you to your first appointment:

  1. 1. Medical insurance card
  2. 2. Photo ID
  3. 3. Sleep study results (if applicable)
  4. 4. Imaging reports (if applicable)
  5. 5. New Patient Packet (only needed if you did not use our online registration system) Any current treatment devices such as night guards or sleep devices

Important: A parent or guardian must accompany all patients under the age of 18 at the consultation visit.

Paying for Your Care

Your treatment may or may not be covered under your medical insurance.

Temporomandibular dysfunction and orofacial pain are considered medical conditions, and related claims are typically filed with medical insurance. Many insurance companies do not cover non-surgical TMJ treatments or orthotic appliances.

Obstructive sleep apnea is a medical condition that can be treated with a dental sleep appliance. Coverage for this is subject to your medical insurance policy and deductible.

Before your appointment, we recommend contacting your medical insurance provider to verify your coverage. To assist you, we’ve created a document with key questions that may help guide your conversation with them. Please fill it out and bring it with you to your appointment.

Additionally, we offer a 10% discount for payment in full at the time of treatment. These savings are limited to patients not utilizing an insurance program. We accept cash, check and major credit cards.
